a) Your physical or electronic signature. b) Identification of the material that has been removed or disabled and its location before removal. c) A statement, made under penalty of perjury, that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification. d) Your contact information, including your name, address, telephone number, and email address. e) Consent to the jurisdiction of the federal district court for the judicial district in which your address is located, or if outside the United States, to the jurisdiction of the federal district court for the District of Columbia. f) A statement that you will accept service of process from the party who filed the DMCA notice or their agent.
- Repeat Infringers: In accordance with the DMCA, we have a policy to terminate the accounts of repeat infringers.
